The route begins from Ulu Klang, Selangor in the north, then runs along the Middle Ring Road 2 to Ampang. The expressway then turns southeast through the town of Ampang, before looping back southwest through Cheras and Alam Damai. Finally, the expressway turns westward to rejoin the Middle Ring Road 2 before terminating at Sungai Besi, where it continues onto the Shah Alam Expressway.

The increasing traffic volume along the FT 28 Kuala Lumpur Middle Ring Road 2 through the connections of Ulu Klang, Ampang, Pandan Indah, Cheras, Bandar Tun Razak, and Bandar Tasik Selatan has created the need for enhanced road capacity. Prolintas proposed the construction of the expressway to meet this demand. The expressway has served as an alternative route to Kajang from Ulu Klang. The project was allocated RM 5.3 billion in the 2015 Budget.

Most of the expressway is elevated above the main road surface. This is the first road in Malaysia to have a spiral ramp and a rest area and service area at Tasik Tambahan.
